You can print ID tickets with the information we need for each sample. Circle the requested service(s) and fill in the information. Put an ID in each sample bag.**

**Effective 4/29/16, official agencies can go outside of their assigned territory in order to pick up submitted samples. If you have the need for our services and would like to be added to an existing route please contact us. We have no monthly commitment fee, & no mileage charge.
